These programs have surpassed initial projections, costing more than $1.6 billion through June 2024. Rapid enrollment, data gaps, and emergency rule changes such as freezes and caps have intensified fiscal challenges, contributing to ongoing budget discussions regarding which benefits will persist and who will qualify.
Capitol News Illinois reported that the audit revealed the programs "far outstripped" estimates, costing Illinois $1.6 billion by last summer. Specifically, HBIA alone incurred expenses of $485.3 million in its first two years—approximately 284% over combined estimates for both programs—while HBIS accounted for hundreds of millions more. These figures have prompted bipartisan scrutiny concerning budgeting and eligibility controls. Lawmakers are considering enrollment caps and new cost controls while scheduling hearings to review oversight by the Department of Healthcare and Family Services (HFS).
The Associated Press noted in May that Illinois' proposed fiscal year 2026 plan included potentially ending HBIA due to budget constraints. At that time, it was estimated that the programs were providing Medicaid-style coverage to over 43,300 immigrants at an annual cost of about $538 million. Enrollment was frozen for adults under 65, with only seniors’ coverage continuing for existing enrollees after June 30. Governor Pritzker's office acknowledged that costs had exceeded projections, leading lawmakers to consider further limitations.
