SWEDISHAMERICAN HOSPITAL: SwedishAmerican Celebrates Opening of New Employer-Sponsored Partners Health Center

SwedishAmerican Hospital issued the following announcement on Jan. 29.

SwedishAmerican welcomed employees from six different locally-owned businesses Tuesday with the opening celebration of the Partners Health Center.

The employer-sponsored Partners Health Center opened Jan. 2 and is located in the same building as our North Main Clinic at 2601 N. Main Street. Partners Health Center offers participating health plan members primary care needs from a single location. The new center is only available to employers who chose to offer access to their employees and/or dependents for their primary care benefit.

“This is another significant achievement for SwedishAmerican,” SwedishAmerican President and CEO Dr. Michael Born said. “Partners is a unique, employer-based clinic specifically designed for Rockford’s locally-owned businesses. It is also one of three clinics opening this year on the west side of Rockford. We couldn’t be more pleased with of all of our growth. As we’ve seen in recent months with the groundbreaking of the Edgewater Medical Center and the opening of the Riverwest Clinic, SwedishAmerican has a long-standing commitment to take care of all community members. No matter where they live. No matter where they work.”

The six participating companies that Partners will serve are Integrated HomeCare Services, Sheet Metal Workers Local 219, Forest City Gear, Rockford Fastener, Spider Company and KB Chiropractic. All are under either Quartz or Employer’s Coalition on Health plans.

“Forest City Gear is so excited for the medical home product we have found in the Partners Health Center,” said Kika Young, Director of Human Resources for Forest City Gear. “I have always admired the medical home model and its emphasis on patient care. With this model, SwedishAmerican and the Partners Health Center provide us access to care in a facility that is dedicated to our employer group, employing best-in-class staff and emphasizing coordinated care.”

Dr. Jennifer Guffey will serve as the clinic’s primary care physician. Services offered at Partners Health Center include:

Family medicine

Laboratory

Chronic disease management

Care coordination and navigation

Wellness education classes

Health coaching

Limited generic prescription medication dispensing

Same day access

Immunizations
