III. RESOLUTIONS

A. Award of Bid: Property Clean-ups to Slabaugh & Sons, of Caledonia, Illinois, Ayers Snow & Junk Removal, of Rockford, Illinois, and CMM & Associates, of Rockford, Illinois for an estimated annual contract amount of $185,000. The contract duration is one (1) year with four (4) one-year extensions. The funding bsource is Construction Services Operating Budget

B. Award of Bid: Fire Department Station Wear to Uniform Den, of Loves Park, Illinois and C & E Specialties, of Rockford, Illinois for an estimated annual cost of $125,000 (based on unit prices on the bid tabulation). The contract duration is one (1) year, with four (4) possible one-year extensions. The funding source is Fire Department Operating Budget.

C. Award of RFP: Emergency Solutions Grant to Remedies Shelter - $50,000 for Emergency Shelter, to Institute for Community Alliances - $30,000 for Homeless Management Information System (HMIS), to Second/First Church - $22,168 for Emergency Weather Operations & Services, to Shelter Care Ministries - $36,300 for Emergency Shelter. The contract duration is two (2) years. The funding source is the Federal Department of Housing and Urban Development (HUD).

D. Award of RFP: Audit Services to Lauterbach & Amen, of Naperville, Illinois for the contract amount:

2020 - $79,500, $125/hour consulting

2021 - $81,900, $128/hour consulting

2022 - $84,400, $131/hour consulting

2023 - $86,900, $134/hour consulting

2024 - $89,500, $137/hour consulting

The funding source is Finance Department Operating Budget.

E. Award of Contract Extension: Tire Collection and Recycling Service to Green Road Tire Recycling, of Rockford, Illinois for a contract amount of $40,000 (based on unit pricing). The contract duration is 1/1/20-12/31/20 of four (4) oneyear extensions available. The funding source is Community Development Operating Budget.

F. Preparation and Submittal of USEPA Grant Application for two City-owned sites, The Rockford Watch Factory and the former Lloyd's Hearing building. The total request is for $275,000, requiring a local match of $55,000.

IV. COMMITTEE REPORTS

A. Approval of Vouchers in the amount of $9,060,222.28

B. Application for Federal grant funds for the annual continuation of Head Start and Early Head Start services for the period of 4/1/20 - 3/31/21.

C. Approval of Self-Assessment Report and Self-Assessment Work Plan for the City of Rockford Head Start/Early Head Start.
