A. Vouchers in the amount of $9,026,721.93

B. Approval of Agreement with Elite Lifeline for Ambulance Services when the City does not have internal resources available to respond to calls for service.

IV. RESOLUTIONS

A. Award of RFP: Well Drilling Consultant (Design Engineering Well No. 46) to Strand Associates, Inc., of Madison, Wisconsin in the amount of $83,000. The project is estimated to be completed by December 29, 2023. The funding source is Water Replacement Improvement Account (WRIA).

B. Award of Non-Competitive Contract: Mechanical Water Meter Supplies to Badger Meter, Inc., of Milwaukee, Wisconsin for an estimated annual value of $300,000, based on unit pricing. The contract duration is through December 31, 2023 with four (4) possible one-year extension options (until December 31, 2027). Annual price adjustments for CPI considered at contract renewal only. The funding source is Water Fund Operating Budget.

C. Award of Non-Competitive Contract: Automated Meter Reading (AMR) Endpoint Supplies and Services to United Systems, of Benton, Kentucky (Itron Region Supplier) for an estimated annual value of $200,000, based on unit pricing. The contract duration is through December 31, 2023 with four (4) possible one-year extension options (until December 31, 2027). Annual price adjustments for CPI considered at contract renewal only. The funding source is Water Fund Operating Budget.

D. Award of Engineering Agreement: Well No. 13 Reservoir Exterior Rehabilitation to Strand Associates, Inc., of Madison, Wisconsin in the amount of $99,000 (includes design and construction engineering). The project is estimated to be completed in 2023. The funding source is Water Replacement Improvement Account (WRIA).

E. Approval of Acceptance of UW Health Swedish American Hospital Mobile Integrated Healthcare Grant in the amount of $125,000. This grant will be utilized to provide ongoing funding for a current mobile integrated health program. This has been a multi-year award to fund an existing position. Both parties continue to see value in this program for reducing 911 calls and hospital readmissions.

F. Approval of Acceptance of Illinois Department of Commerce and Economic Opportunity to receive a Community Services Block (CSBG) Grant Award in the amount of $1,035,456.00. The grant does not require a cost match and the grant term is from January 1, 2023 to December 31, 2023. Grant Funds will be used for developing and implementing programs to assist low-income Illinois residents in working towards family stabilization and self-sufficiency in furtherance of this purpose.

G. Acceptance of Centers for Disease Control and Prevention's, Closing the Gap with Social Determinants of Health Accelerator Plans Grant Award in the amount of $124,961. Grant funds will support staff, training and collaborative community conversations. The Project Coordinator hired under this grant, will be able to coordinate with the existing front-line program staff and management teams within the divisions to aid in the overall planning efforts.

H. Approval of Tax-Exempt Lease for 2 Fire Engines and Outfitting to Midland States Bank for a lease rate of 2.85%. The lease will require annual payments of $221,447.66 for seven years.

I. Resolution Authorizing the Central Services Manager to execute a contract for 2022-2023 Medium-Duty Fleet for the City on terms as deemed appropriate and in the best interest of the City.
