B. Emergency P.O. - City Yards Fuel Tank Repair to Stenstrom Petroleum Services of Rockford, Illinois in the amount of $33,831.00. The funding source is Property Fund Operating Budget.

C. Change Order: Gregory Heights Drainage Improvements (Phase 2) to N-TRAK Group of Loves Park, Illinois

Original amount: $639,056.70

Change order: 186,438.21

Amended Amount: $825,976.32

This change order includes storm sewer manhole upsizing, additional tree removal, additional curb and gutter removal and replacement, additional pavement removal and replacement, additional approach pavement removal and replacement, additional sidewalk removal and replacement, and increased material cost due to supply chain interruptions necessitating the procurement of alternate materials that were available at the time of construction. The upsizing of manholes was due to unexpected underground utility location conflicts and existing underground storm sewer connection angles being larger than anticipated.

D. Change Order - City Wide Sidewalk Repair 2022 Package #1 to N-TRAK Group of Loves Park, Illinois

Original amount: $233,472.50

Change order: 63,099.70

Amended amount $296,572.20

The City Council awarded the construction contract for the CW Sidewalk Repair 2022 Package 1 on March 28, 2022 to N-TRAK Group. The work was completed on July 15, 2021. Additional work was requested at the locations listed below. The following work includes tree root pruning, curb and sidewalk removal and replacement. Due to similar scope of work, close proximity to the original project sites and the competitive pricing received, the Public Works Staff felt it was in the best interest of the City to add the following work to the package.

∙ 3200 block of Dartmouth Drive

∙ 700 block of Lundval Avenue

∙ 500 block of Roland Avenue

∙ 200 block of N. London Avenue

∙ 814 20th Avenue

∙ 2329 8th Street

III. COMMITTEE REPORTS

A. Vouchers in the amount of $5,049,267.12

IV. RESOLUTIONS

A. Award of Bid: Guardrail Installation and Repair Services to Northern Contracting, Inc. of Sycamore, Illinois and Huskie Contractors, LLC of Byron, Illinois for an estimated annual contract value of $40,000.00 (based on unit pricing). The contract duration is through April 30, 2023 with four (4) possible one-year annual extensions. Annual price adjustments for Consumer Price Index (CPI) considered at contract renewal only. The funding source is Street Division Operating Budget.

B. Award of Bid: Seminary Street Resurfacing (North of Catherine Street to south of Bremer Street) to Rock Road Companies of Janesville, Wisconsin for $844,804.75. The project is estimated to be completed by October 14, 2022. The funding source is 1% Infrastructure Sales Tax.

C. Award of Bid: City Wide Street Repairs Group No. 5 - 2022 (Parking Lots) to N TRAK Group of Loves Park, Illinois in the amount of $1,447,553.77. The project is estimated to be completed by October 28, 2022. The funding source is ARPA Revenue Replacement Fire Department ($832.034.67), ARPA Revenue Replacement Street Department ($327,926.35) and Parking Fund Bond Proceeds ($285,592.75).

D. Award of Bid: Bridge Repairs 2022 to Martin & Company of Oregon, Illinois in the amount of $148,628.00. The project is estimated to be completed by October 28, 2022. The funding source is 1% Infrastructure Sales Tax.

E. Award of Contract(s): Public Works Materials to Northern Illinois Service of Rockford, Illinois, William Charles Construction of Loves Park, Illinois, Rock Road Companies of Janesville, Wisconsin and N-TRAK Group of Loves Park, Illinois for an estimated annual contract value of $200,000.00 (based on unit pricing). The funding source is various departmental operating budgets.
