Mercy Rockford Health System issued the following announcement on April 3.
On Tuesday, April 16, 2019, Mercyhealth, along with Crusader Community Health and Prairie State Legal Services, will host an open house for National Healthcare Decision Day.
The event is designed to explain a person’s health care decision options and what they can do in the event they are unable make their own health care decisions.
Attendees will learn about these forms:
Healthcare Power of Attorney
POLST (Provider Orders for Life-Sustaining Treatment)
An attorney from Prairie State Legal Services will meet with individuals at no cost. These legal services are funded in part by a grant from the Community Foundation of Northern Illinois. Attendees can privately discuss advance planning options and learn about forms that spell out their health care wishes if they are no longer able to do so.
Members of Mercyhealth’s hospice care, palliative care, and Advanced Illness Management (AIM) teams will also be present to educate participants on the right to life choices available for Advance Directives. Members of Prairie State Legal and Crusader Community Connections staffs will be also available for this discussion.
While the focus of this open house is on Healthcare Power of Attorney information, those wishing to seek information regarding a Financial Power of Attorney can schedule a future appointment by calling Crusader Community Connections at (815) 490-1680.
